At the Dr. De LLanos Clinic, your treatment begins
with the staff. They are there to assist in scheduling
new patient and follow-up appointments,
answer questions about Life Extension vitamins and supplements, process
orders for Life Extension products, relay detailed messages to the
Clinicians, and answer questions about
insurance coverage and co-payments.

For new
patient appointments...
The following information is needed when a new
patient appointment is being scheduled:
~ Name of caller
~ Name of patient (if different than caller)
~ Best phone number(s) to be reached
~ Date of Birth of patient
~ Current symptoms and issues being experienced by new patient (please
provide as many details as possible as this is extremely helpful
to
Dr. De LLanos, Joselee Sarmiento, NP, Deborah Ellis, NP, Patricia
Soderlund, NP, and the other Clinicians in scheduling and during an initial appointment)
~ Current medications and/or medications tried and failed
~ Referral source
~ Insurance carrier, subscriber identification number, group number (if
applicable), insurance customer service phone number and Authorization number (if applicable).
PLEASE BE AWARE: If insurance information is not provided upon
scheduling of your initial appointment, you will be expected to pay the
full fee for service as all insurance benefits need to be verified prior
to arrival of your first appointment.
